• A cleared completed screening form is required for service attendance.
• You will be asked to comply with all rules and regulations.
• You will need to complete the pre-screening form which is due by Thursday each week.
• Your temperature will be taken upon arrival.
• You will need to sit in designated areas only.
• You are required to wear a mask throughout the service, covering your nose and mouth.
• Cloth masks must be worn with other filtered masks.
• Please sanitize your hands after each contact.
• Please maintain 6 feet distance from each other, unless you are from one family.
• Please do not congregate in the lobby.
• You are required to use the appropriate entrances and exits as requested.
